This is an online eblog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio.   CASESHEET  :A 42 year old woman has come to hospital with complaint of Fever, involuntary moments CHEIF COMPLAINTS : Fever, involuntary movements. HISTORY OF PRESENT ILLNESS : (i)The  Patient  had symptoms of cold,cough  10 days ago (ii)Then she was affected by fever       FEVER : Low grade intermittent diurtional           variation at night time  (iii)Chills at night (iv) Body pains and headache during night (v) Vomitings : Food particles (vi) Stools : Loose stools ( watery) since 4 days (vii) Seizures: 5 episodes of partial seizures   HISTORY OF PAST ILLNESS : Gall bladder was operated six years ago Asthma : No Asthma Tuberculosis : No tuberculosis CAD : No CAD CVA : No CVA BP : High BP Thyroid : Hypothyroidism  DRUG HISTORY : Amlodipine & Atenol

CHEIF COMPLAINTS : Unable to walk like normal. HISTORY OF PRESENT ILLNESS : (i)The  Patient has suddenly was unable to walk and he couldn't walk since 4 weeks (ii)He was feeling the tingling sensation and sweating all over his legs after he found that he couldn't walk normally (iii)It was then followed by pedal edema ( iv) legs became weak eventually    HISTORY OF PAST ILLNESS : No significant past history  DRUG HISTORY : After admitting in the hospital he was given the thyroid drug. This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.   CASESHEET :A   50 year old man has come to hospital with complaint of cessation of urine. CHEIF COMPLAINTS : Cessation of Urine since 9 days along with legs pain and edema HISTORY OF PRESENT ILLNESS :The  Patient has sudden stoppage of urine asymptomatically, then consequently the patient used to suffer insidious bilateral pedalgic & pedaledema(pitting edema) and he also used to get backache.He also got sometimes low grade insidious fever. Associated diseases : Hypertension  Diabetic   HISTORY OF PAST ILLNESS : (i) The patient was suspected Diabetic   12 years ago. (ii) He has undergone the Hernia operation 12 years ago. (iii) He also undergone the Piles operation 5-6 years ago. (iv) The right eye vision is Blur vision. (v) Radiating Headache throughout

CASE HISTORY : A 23 year old known case of Seizures CHIEF COMPLAINTS : seizure,fever, Headache  HISTORY OF PRESENT ILLNESS :  Patient was suddenly affected by seizures followed by fever and Headache Seizures: patient was attacked twice Fever : Diurnal variation with fever at night times,Low grade fever Headache: overall radiating pain all over head PAST ILLNESS : No similar complaints  PERSONAL HISTORY :     Diet: Mixed     Appetite: No appetite      Sleep: good     Bowel movements : regular     Micturation : Good     Addictions:No FAMILY HISTORY: no history of similar complaints in the family  GENERAL EXAMINATION: Patient was moderately built and moderately nourished Pallor-normal Icterus-normal Cyanosis -normal Clubbing - no clubbing   Edema -no edema
