Case scenario.....
Hi, this is Swarna Kurma, IIIrd BDS. This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.

 CASESHEET :A 50 year old man has come to hospital with complaint of cessation of urine.


CHEIF COMPLAINTS: Cessation of Urine since 9 days along with legs pain and edema

HISTORY OF PRESENT ILLNESS:The  Patient has sudden stoppage of urine asymptomatically, then consequently the patient used to suffer insidious bilateral pedalgic & pedaledema(pitting edema) and he also used to get backache.He also got sometimes low grade insidious fever.



Associated diseases:
Hypertension 
Diabetic

 
HISTORY OF PAST ILLNESS:
(i) The patient was suspected Diabetic  12 years ago.
(ii) He has undergone the Hernia operation 12 years ago.
(iii) He also undergone the Piles operation 5-6 years ago.
(iv) The right eye vision is Blur vision.
(v) Radiating Headache throughout the scalp
(vi) Left knee cap pain due to injury 


DRUG HISTORY:

PERSONAL HISTORY
Diet : Mixed
Appetite : No appetite 
Bowel movements : loose motions only when the prescribed syrup is drank.
Bladder movements: micturition since 2 days
Addictions : Alcohol 
Known Allergies : nil

FAMILY HISTORY
No significant family history 

GENERAL EXAMINATION:
Patient is Conscious nd Co-operative 
Built : Moderately built
Nourishment : Moderately nourished
Pallor : No 
Icterus : No
Cyanosis : No
Clubbing : No
Lymphadenopathy : No
Edema : pedaledema previously present not now

VITALS:
Temperature: 98.6°c/f ( afebrile)
Pulse: 92 beats per minute
Respiratory rate: 16 cycles per minute
Blood pressure: 160/100 mm of Hg
SPO2: 100%
GRBS: 310mg%

LABORATORY DIAGNOSIS
Questions:
1.What caused the sudden stoppage of urine?
2.What happened to inside the patient body when he did not urinate about 9days?
3.Can he get cured permanently?
4.Can he get cured only through medications or surgery is required?
