CASE scenario....

Hi, I am Swarna Kurma, 3rd year bds student. This is an online elog book to discuss our patients health data after taking her consent. This also reflects my patient centered online learning portfolio.

CASE HISTORY : A 23 year old known case of Seizures
CHIEF COMPLAINTS : seizure,fever, Headache 

HISTORY OF PRESENT ILLNESS : 
Patient was suddenly affected by seizures followed by fever and Headache
Seizures: patient was attacked twice
Fever : Diurnal variation with fever at night times,Low grade fever
Headache: overall radiating pain all over head

PAST ILLNESS :
No similar complaints 

PERSONAL HISTORY :
    Diet: Mixed
    Appetite: No appetite 
    Sleep: good
    Bowel movements : regular
    Micturation : Good
    Addictions:No

FAMILY HISTORY: no history of similar complaints in the family 

GENERAL EXAMINATION:
Patient was moderately built and moderately nourished
Pallor-normal
Icterus-normal
Cyanosis -normal
Clubbing - no clubbing 
 Edema -no edema
 
VITALS
Temperature -98°F
Pulse-96 beats per minute
Bp-120/80mm of hg 
Spo2-98%

                                Q/A
1.why did she affected suddenly by the      seizures?
2.Can it be cured permanently?

This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.   CASESHEET  :A  27  year old man has come to hospital with complaint of Unable to walk. CHEIF COMPLAINTS : Unable to walk like normal. HISTORY OF PRESENT ILLNESS : (i)The  Patient has suddenly was unable to walk and he couldn't walk since 4 weeks (ii)He was feeling the tingling sensation and sweating all over his legs after he found that he couldn't walk normally (iii)It was then followed by pedal edema ( iv) legs became weak eventually    HISTORY OF PAST ILLNESS : No significant past history  DRUG HISTORY : After admitting in the hospital he was given the thyroid drug.
